The First Contact Practitioner works within specific GP surgeries to support you with your mental health, working in a collaborative way to meet your needs.
Who is it suitable for?
Anyone who is 18 and over and is registered with one of the following GP surgeries:
- Amersham Health Centre
- Rectory Meadow Surgery
- John Hampden Surgery
- Prospect House Surgery
- Hughenden Valley Surgery
What can I expect?
Our First Contact Practitioner, acting as a Mental Health Worker, will conduct a 20-minute triage assessment to evaluate your needs. Following the assessment, we will either refer you to the most suitable service or provide access to our guided self-help program. This ensures that each individual receives the best possible care, based on their situation.
If appropriate, the Practitioner may recommend guided self-help, offering structured support to help you manage your mental health independently.

There is no charge for this service.
